Please complete the application form below if you would like to apply for AOIC membership.
This is a multi-page form and you can save your progress as you go along.
Before beginning your application, please try to have the following ready to input/upload to the form (if you do not have these items available at present then you are welcome to complete the form and send them over by email at a later date):
- your completion of training certificate or another relevant document
- a personal bio for your directory listing (approx. 200-300 words)
- a suitable photograph of yourself for your directory listing and ID card.
